How Do I Apply?
In order to be able to apply for ADP funding you need to be assessed by a therapist who is registered as an "authorizer" by ADP. The authorizer will complete the application for the devices needed and forward it to ADP. Astra Healthcare can help you to find an ADP authorized Therapist.
What is the Assessment?
At the assessment, the authorized therapist will assess your condition and will determine how you will benefit from a mobility device, and which mobility device(s) will best fit your needs. He / She also will then determine the size, model, and features of the equipment. Astra Healthcare will provide the necessary equipment for the assessment.
After the Assessment
Authorizer will fill the form and ask you to sign it and will send it to ADP. It takes some time for ADP to process the request. Meanwhile Astra Healthcare can provide you with a rental / Loaner. When you are approved ADP will notify the authorizer as well as Astra Healthcare (Your vendor of choice). Astra Healthcare will provide you with the equipment and you’ll pay your portion.
